POSITION SUMMARY

Responsible for the assessment and triage of children of all ages and for coordinating the deliver of care. As an active member of the multidisciplinary team, facilitates those functions associated with arranging care and providing referrals. Represents Children's Health to patients, families, potential referral sources, agencies and payers from initial inquiry through discharge from care. Must be knowledgeable of clinical aspects of care for patient population including evidence-based assessments, treatments and research methodologies. Provides patient-centered care with a strong focus on customer service.

ESSENTIAL FUNCTIONS AND JOB SPECIFIC RESPONSIBILITIES:

Requires in-depth professional knowledge and practical/applied expertise in own discipline and basic knowledge of related disciplines within the broader professional field

Has knowledge of best practices and how own area integrates with others; demonstrates awareness of the industry, including regulatory, evolving customer demands, and the factors that differentiate the organization in the market

Acts as a resource for colleagues with less experience; may lead projects with manageable risks and resource requirements

Solves complex problems and takes a new perspective on existing solutions; exercises judgment based on the analysis of multiple sources of information

Coordinates and provides clinical triage that is responsive to patient and family needs. Determines whether pataient's benefits provide coverage for services and communicates this information back to families. Works with clinical team to develop guidelines and criteria for triage, and contributes clinical expertise regarding appropriate services or care. Educates patient families regarding programs, assessment process, scheduling, and billing.

Promotes the integration of research and clinical care by providing information to patients, families and referral sources regarding opportunities to participate in research.

Collaborates with care team to determine care needs; follows up with patients, families, physicians, care team members, fnancial and support staff to coordinate clinical services and improve the patient experience. Works collaboratively with other departments and services. Participates in the development, implementation, evaluation and revision of programs and initiatives to improve quality, continuity of care and cost effectiveness.

Develops an internal and external referral resource directory and provides referral advice to patients, families and providers. Consults with patients, families and providers to determine additional referral resource needs. Serves a a liaison to community resources. Provides feedback to referral resources to enhance continuity of care.

Creates and maintains a database for service analysis and program development.

PATIENT CONTACT

Direct - Indicates that the person in the position directly provides care, treatment or services to the patient. Ex: Nurses, Physical/Occupational Therapists, RCPs.

EXPERIENCE

3-5 years of related experience, required.

EDUCATION

Four-year Bachelor's degree or equivalent experience, required.

Graduate or professional work or advanced degree; or equivalent experience, preferred.

LICENSES and CERTIFICATIONS

Texas license required: RN, required.

PHYSICAL DEMANDS

Light - Exerting up to 20 lbs. occasionally, 10 lbs. frequently, or negligible amounts constantly and may require walking or standing to a significant degree.
