Summary

Supervises and coordinates activities necessary to ensure that the Grand-Aides program is implemented and administered in accordance with established guidelines and contract requirements. Trains, supervises, instructs and guides Grand-Aides in the day to day operations of the program. Conducts assessments and patient teaching. Utilizes nursing process to achieve optimal patient care and meet program goals. Communicates patient findings to providers and follows through with provider orders/recommendations via written and verbal communications. There is ZERO DIRECT PATIENT CARE in this position. This position will be located in various Children's Health Pediatric Group Clinics (Cockrell Hill, Cedar Hill, and Oak Cliff) as well was at the administrative offices at Trinity Towers in Dallas.

Responsibilities

  • Develop and deliver training to Grand-Aides; assess learning; modify training as needed; evaluate the quality and effectiveness of curriculum; evaluate performance of individuals post training; assess learning needs ongoing and develop and deliiver in-services
  • Assess patients in the program, understanding current medications, treatments and therapies; revies Grand-Aides' documentation and determine any needed interventions; and instruct Grand-Aides in completion of their documentation
  • Collaborate with the Medical Director/Program Director to develop policies and procedures that support the program; recruit and select Grand-Aide staff; provide leadership, coaching, clinical supervision and evaluation of Grand-Aide staff; assign client caseloads and monitor Grand-Aides' abilities to manage and build caseloads; evaluate overall Grand-Aides' performance quarterly duirng year 1, semi-annually year 2, and annually thereafter; assess competencies and address and resolve performance issues
  • Assist in developiing, revising and evaluationprogram goals; collaborate with leadership to ensure program compliance
  • Review clinical record documentation and outcomes with Grand-Aide staff; hold supervisory patient conferences with staff to identify revisions needed to treatment plans or goals; analyze data and create reports to assess and report on care quality and status of program goals

Education, Experience, Certifications

  • 3-5 years of clinical experiences as an RN or APN - REQUIRED
  • 2 or more years of experience leading a team - preferred
  • Bilingual in Spanish - preferred
  • Current RN or NP license in the state of Texas - REQUIRED
  • Current CPR Certification - REQUIRED
