The Grand Aide acts as a nurse assistant to an RN and conducts homve visits to improve or maintain the patient's health. The Grand-Aide completes a checklist of educational topics, documents the patient's environment and general condition. The connects to an RN during the home visit via a virtual platform to get advice as needed to improve the patient's health and/or reduce the need for more costly medical interventions. Community Health Workers, Medical Assistants, and Certified Nurse Aides who wish to work with our target populations are encouraged to apply to be Grand Aides. Grande-Aides need to be comfortable completing home visits, educating patients and families, and being in the community they serve. The Grand Aide position is a home health visit aimed at working with healthy children; Grande Aides will use their time to educate care givers using materials supplied to them.
