Position Summary:

This job is responsible for preparing the paper medical records for scanning, scanning and indexing of documents and performing quality audits on scanned documents.

Job Specific Responsibilities:

  • Prepare documents daily for scanning and indexing.
  • Perform scanning process on completed documents.
  • Index scanned documents to the correct patient account and document type.
  • Perform quality audits on documents scanned into the document imaging system.
  • Maintain accuracy and productivity standards to ensure accurate and timely completion of work.
  • Answer telephones and assist as needed.
  • Complete data entry as necessary in various aspects of the department; perform additional clerical duties and special projects as requested.
  • Complete daily productivity reports.
  • Index and box records for storage.
  • Attend and participate in in-service training and continuing education related to job responsibilities.
  • Perform other duties as assigned to include preparing documents for release of information.

Patient Contact:

  • Indirect - Indicates that the person in the position is responsible for a product (including information) or service that is involved in the patient’s course of care, treatment or service. Ex: Clinical Social Workers, Patient Access Representatives, Language Access Services Representatives


  • 3-4 years of medical record experience or related education / certification required
  • 2 years of Windows skills or document imaging experience required
