POSITION SUMMARY: Disease Management (Weight Mgmt)

Implement and administer policies, programs and activities. Responsible for performing psychosocial assessments on families’ ability to meet their child's healthcare needs. Ensure the preparation and maintenance of required documentation per accreditation standards and CMC's policy.

Member of the primary care team and in communication with various disciplines to ensure social barriers to care are met and/or addressed. Patient Interaction is predominately telephonic, with in-person interaction occurring 15% of the time.

Additional job responsibilities:

Aids in problem solving with family as it relates to financial stressors, e.g., meal assistance, non-ambulance transportation, prescription assistance, housing, etc.

Requires occasional travel (15%) to meet with patients in their home and/or to designated locations, e.g., local YMCA

Provides assessment and intervention when there is suspicion of family violence, parental discord, custody concerns

Further assist patient/families with no payor source

Addresses barriers to follow-up appointments, medication adherence, etc.

Experience charting in an electronic medical record


Requires experience in own professional discipline to appropriately apply own knowledge; broadens own professional expertise and acquires higher level skills

Understands key business drivers; uses this understanding to accomplish own work

No supervisory responsibilities but may provide informal guidance to new team members

Solves problems in straightforward situations; analyzes possible solutions using professional/technical experience, judgment and precedents

Impacts quality of own work and the work of others on the team; works within guidelines and policies

Perform biopsychosocial screening and assessment of the families’ ability to meet their children's healthcare needs by applying specialized knowledge and advance practice skills in assessment treatment, diagnosis of mental illness, planning, implementation and evaluation.

Facilitate parent/child adjustment to illness and hospitalization.

Coordinate anticipatory discharge to ensure that patient's home - care is prearranged prior to leaving the hospital.

Coordinate and direct multidisciplinary patient care conferences focusing both on medical and biopsychosocial needs.


Two to three years of previous Social Work Experience Required

Four to five years of related experience Preferred


Graduate or professional work or advanced degree; or equivalent experience Required


State of Texas LMSW license Required

State of Texas LCSW license Preferred
