Provide nutrition education with a focus on expanding and sustaining healthy lifestyle behaviors in patients and their families. Applies critical thinking and knowledge of evidence based practices to develop and assist in the development of public health programs. Project manages day-to-day operations for select community-based health programs.
Ensure patients, families, clinic staff, and the community receives the best nutritional information available. Builds relationships and fosters collaboration with external partners to serve patients and their families.
Whenever there is a need, will use clinical skills and ability to work with multidisciplinary teams to plan, coordinate, document, and communicate aspects of a patient's health management needs. Facilitates appropriate utilization of services if needed.
